Carer Information

The primary and secondary carers are normally the parents of the child however this is not always the case. The primary carer will always be the first point of contact we use when regarding your child, if they can't be reached then the secondary carer will be contacted. Please provide information for both your primary and secondary carer.

If there are any changes to the information provided below (or if any such issues come to light after you have registered with us) then you must provide us with as much notice as possible so that we can continue to care for your child effectively and safely.

48h sickness & 24h temperature policy*

I/we understand that the nursery has a 48 hour sickness policy and a 24 hour temperature policy. If my/our child is suffering from diarrhoea or vomiting I/we will not bring him/her back to nursery until 48 hours after the last symptom has gone. If my/our child is suffering from a raised temperature I/we will not bring him/her back to nursery until 24 hours after the last symptom has gone. Please do not mask your child’s symptoms by Calpol and bring into nursery because the effects wear off after 4 hours and we shall have to call you back!

I/we also understand that if my/our child is unwell during the day the nursery will contact me/us or our emergency contact so my/our child can be collected as soon as possible

Medical emergencies*

I/we give my/our consent for a senior member of Greenwood’s staff to deal with any medical emergency that may occur to my/our child and to seek the appropriate medical advice or treatment that may be necessary.

It is important that you read and understand all of the school’s policies and procedures. All policies and procedures are reviewed and updated throughout the year.

15/30 hours government funding

The government offers 15/30 hours funding for under 2's (9-24 months), 2 year olds (24-36 months) and 3-4 year olds (36- 60 Months).

The funding is offered for 38 weeks of the year. This is split over three terms. 14 weeks in the Autumn Term (Sept-Dec). 12 Weeks in the Spring Term (Jan- March) and 14 weeks in the Summer Term.

This funding only covers basic care and does not cover the following; consumables (nappies, wipes, sunscreen, toothpaste etc.), Food costs (breakfast, lunch and tea) and extracurriculars costs (tennis, yoga, dance, drama, music, forest school and intergenerational learning).

If you want these additional services included in your package then you must agree to our additional package services. The cost of these packages vary depending on which rate you are on and only cover the difference between the funded hourly rate and Greenwood's hourly rate.

If you choose this option, your funding will be spread across 51 weeks rather than 38 weeks. Therefore, if you are eligible for 30 hours funding this equates to 22.4 hours per week. If you are eligible for 15 hours funding, this equates to 11.2 hours per week.

If you only agree to the basic rate then you will NOT have access to the extracurricular activities listed above and you will need to bring your own food into school and MUST comply with our Packed Lunch/Tea Policy. Any additional days you want to take after your 38 weeks are charged at the normal rate.

If you agree to the basic rate and don't attend outside funded weeks your place WILL be secure for all 38 weeks of term-time.

It is important that you read and understand the Terms and Conditions attached to this Registration Form as they help you understand our relationship with you so that we can make sure your child is cared for as effectively as possible and provide relevant consents in connection with our Services. By signing this Registration Form, you agree to the attached Terms and Conditions, which are incorporated into your agreement with us for providing our Services to your child. The agreement between us is binding if signed by either parent/guardian, or both parents/guardians, as identified on page 1 of this Registration Form. We would suggest that you keep a copy of this Registration Form with the Terms and Conditions for your own records.

I/we understand that three full calendar months’ notice in writing is required if I/we wish to remove our child from the nursery permanently.

Photograph consent*

I/We hereby agree to the nursery taking photographs of my/our child, for use on the children’s learning journey (using Famly software), display boards, and class group photo, etc. This consent does not include use of photos on the website.

Outings*

As part of our weekly schedule the over 3's visit a care home using our minibus as transportation and their forest school sessions will also occasionally require minibus transportation. As well as this all children will regularly visit the local library, post office and shopping centre by foot. For all other outings the primary carer will be sent a detailed consent form which has to be agreed to grant the child permission to attend.

Face paint

We offer a birthday party service at the nursery which includes face painting. We use Snazaroo an easily washable and skin friendly product. If you are happy for us to offer face painting to your child at special events please sign below

Start up pack*

When starting Greenwood you will be issued with a start up pack at the cost of £100.00. This includes a school smock, a school rucksack and school kit bag ,rental of our school forest school waterproofs and administrative costs for Famly.

In the interest of safety and security we require parent permission for the publishing of children’s work, photographs or videos through a software program called Famly.

By agreeing to this form you grant permission for us to photograph or video your child for the purposes of sharing this information with you through the Program. You will also receive updates and information about your child through the Program to the email you have provided herein. Note that sometimes other children in the school may feature in photos, videos or stories of your child.
